Immunizations

For the safety and well-being of all in our schools, all students in Oak Harbor Public Schools are required by law (RCW 28A.210.020-040) to have a completed Certificate of Immunization Status (CIS) form indicating that they are up to date and current with immunizations, in the process of getting immunizations, or are exempt from getting immunizations.

Students without a completed CIS form will receive “conditional admittance” to school but must submit a completed CIS form within 30 days or be subject to exclusion from school as required by law.
