Each year, the Oak Harbor Youth Coalition conducts the Community Prevention and Wellness Initiative Community Survey in Oak Harbor. The brief survey takes about 10-15 minutes to complete and helps the Coalition better understand opinions about youth substance use in the Oak Harbor community.  Your responses help the Coalition inform and improve prevention programs for the youth in our community.

Who should participate?
Adults who interact with youth regularly – parents, school staff, family, youth-serving organizations, etc. The survey is completely anonymous.

When should I take the survey?
As soon as you are able. It will only take 10-15 minutes and is completely anonymous. The survey period ends on Tuesday, Dec. 15, 2020.
