Immunization Requirements

Immunization requirement for 6th graders - New requirements in 2007.

Washington State Law requires that children attending kindergarten through twelfth grade MUST have an immunization card on file at school.

You must present, on or before your child's first day of school, proof of: 1) full immunization, or 2) initiation of the schedule of immunization, or 3) a certificate of exemption (religious, personal, or medical). Proof constitutes completion of the Certificate of Immunizations Status Form (form) and your signature.
